Transitional Living Program (TLP) Overview ... 751-8223
Evergreen House does not currently provide a residential TLP.

Evergreen's Transitional Living Program is for teens and young adults between the ages of 16 and 21 who need housing and independent living skills to help them successfully transition to adulthood. These services may be available to you if you are homeless, a runaway, have been kicked out by your parents, are unable to live at home, are leaving a foster care placement and have nowhere to go, or are exiting a correctional facility and have nowhere to go.

Some teens and young adults need the full range of services provided by TLP, while others may need only some of TLP's services. All of TLP's services are free to teens and young adults. Some of the services available to you are described below.

  • Off-site housing assistance (subsidies) to help you pay for your own apartment and case management services to help you learn to manage on your own.
  • Independent living skills groups.
  • Job readiness and job search assistance.
  • SELF Program help (click on the button to see if you're eligible for help obtaining a driver's license, tribal or state identification, work clothes, a bicycle to get to work, application fees for college or the tech school, and more).
  • Mental health counseling.
  • Educational assistance - help obtaining your high school diploma, completing your GED, or help enrolling in college or vocational training.

Please be aware that you will be asked to complete an "intake packet & life skills assessment" for several of the services listed above. The information you provide helps TLP staff evaluate how well we may be able to serve you. For more information on TLP's services, click specific choices under the TLP menu button at the top of this page.
