Residential Program 751-2466
TLP's
residential program is licensed under "Rule 8" by the State of Minnesota Department
of Human Services and maintains Title IV E Eligibility. This program is centrally
located in the Bemidji Community and provides transitional services to youth
ages 16 - 19 (maximum of 6 youth at a time.) All youth in the residential program
have access to:
- A case manager who
coordinates services.
- An initial skills assessment
upon entry into the program to determine their level of knowledge in the
various life skill areas. After this assessment is scored and reviewed by
both the youth and the case worker, a transitional living plan (TL plan)
is created. The TL plan acts as a contract between the youth and TLP, establishing
both short-and long-term goals, along with a time line for completion of
goals.
- Evening activities
at the TLP House are designed to address differentvarying aspects of successful
independent living. The typical schedule for evening activities is as follows:

TLP foots the bill for activities such as movies, bowling, etc.

Planning / Grocery Shopping
- Evening meals at the
House are planned and prepared by youth in the program with assistance from
staff. This effort is considered part of their life skills training, and
youth are encouraged to prepare meals that are both nutritionally balanced
and require basic cooking skills.
- Positive involvement
from a youth's family is encouraged.
- Services provided by
Evergreen's Youth and Family Counselor are available to all residents.
- A monthly progress
report is generated for each youth, with a copy sent to both their placing
agent and custodial parent.
